Registration

Download and complete the registration form below to register your school's participants in the 2006 MBAT. Return the completed form to your MBAT coordinator. Note that no refunds are possible after May 15.

Each MBAT participant must provide the following documents at registration on Thursday, May 25:

• signed medical responsibility waiver (below)
• doctor-issued medical clearance certificate (provided by a licensed physician and attesting to the participant's health and ability to compete in his/her chosen sports)
• signed code of conduct (below)

Please be advised that registrants who do not provide these documents cannot take part in the sporting events.
